About the Connected Care Clinical Innovation Grant:

The Connected Care Clinical Innovation Grants directly supports clinician-driven projects, nationwide, that accelerate digital health transformation and interoperability. This year 2025, the program supported clinical leaders within three distinct focuses: Innovation Grants, Artificial Intelligence (AI) Scribe Technology Grants and Indigenous Led Innovation Grants.  In alignment with the Interoperability Roadmap, the Innovation Grants advanced digital health transformation through innovation. AI Scribe Technology Grants allowed creative adaptation of the use of scribes beyond Primary Care and beyond traditional documentation use cases. Dedicated Indigenous funding allowed First Nation communities to identify priorities defined by Indigenous voices and progress as they define it while centered in their perspectives, priorities, and cultural contexts.

Presenter: Liam Venables is the Home and Community Care Manager with Dilico Anishinabek Family Care, based in Thunder Bay, Ontario. A proud member of Fort William First Nation, he is committed to delivering culturally respectful, community-focused healthcare. As a Registered Nurse, he brings strong clinical expertise and leadership to supporting individuals and families in their homes. Liam is passionate about improving health outcomes and advancing equitable care for First Nations communities.
